Abstract
Open AccessThe National Health Service (NHS) in England provides funding for social prescribing to address the social determinants of ill-health. This paper describes a case study of how one general practice serving a disadvantaged population transitioned to Focused Care, a more intensive form of social prescribing, to increase engagement and improve outcomes. Characteristics of service users and their presenting problems, rates of engagement, resource implications of the service and potential benefits are discussed.
