Open AccessThis study provides one of the most extensive long-term follow-up analyses of unplanned revisions following 3CO for spinal deformities, with a mean follow-up duration of 9.8 years, which has been underexplored in prior research. The findings provide critical data for preoperative discussions with patients and their families regarding the risks of unplanned reoperation. Additionally, the study highlights the need for long-term surveillance and proactive strategies to mitigate revision risks, particularly in patients undergoing multilevel 3CO.
