Open AccessLarge trochlear osteochondral lesions are challenging to treat because of the morphology of the patellofemoral joint, and there are multiple technical considerations that are critical to optimizing conformity at the graft site. In certain cases with large or ovoid lesions that are not amenable to transplantation of a single cylindrical osteochondral allograft plug, it may be necessary to transplant multiple overlapping grafts. The purpose of this report is to describe a technique for multifocal osteochondral allograft transplantation with overlapping grafts for the treatment of large or ovoid trochlear osteochondral lesions, often colloquially referred to as the "snowman" technique.
